It has come to our attention that many family members, friends and inmates are not aware they can request a CDC form 7385 which the inmate fills out letting their loved ones on the outside have access to their health care and authorize that person to speak to medical staff about their situation.
You must fill out this form completely and turn it in each time you go to a new facility. Again, to reiterate each time you are moved to a new facility you must then fill out this form again and turn it in…it will NOT BE transferred to the new facility with you. You turn this form into the medical staff; make sure they review it, tell you that it is done correctly and is accepted.
*Remember once you’ve done this that person will have access to your personal Information.
If it’s possible to have a copy of the form made so that you can send it to the person you’ve chosen it is a good idea though not all places will do this and are not required to. As soon as you turn the form in, instruct your chosen person to call the medical release line at your facility to confirm it has been processed and they now have access to your medical information. They will need to provide your full name, CDCR number, birthdate and their phone number and address. It may take a few times to get through but tell them not to give up but to keep trying.
